Why can't I build a Settler in my new city?

Settlers need the city to be at least size 2.

Why did my second policy cost more than I expected?

Policy cost rises with every policy adopted and with every non-puppet city beyond the first. See the policy cost calculator.

Are ruin rewards random?

Yes, chosen from the list above among those whose conditions you meet.
